Athletic Medical Clearance Process for Veteran (Returning) Varsity Athletes

Veteran (returning) varsity athletes are required to complete an 鈥淎nnual Veteran Sports Physical鈥 with 91精品 Health Services.  If you missed your appointment during the spring semester or are re-joining a team then please call Health Services to schedule an appointment for your 鈥淎nnual Veteran Sports Physical鈥 at  860-832-1926
Once your appointment is scheduled you will receive the associated annual Health Service forms for you to complete on-line in .   

Please Note: 
To be medically cleared for sport you must have an up-to-date tetanus (valid for the entire upcoming athletic year) and results of your sickle cell test lab results.  It is your responsibility to up-load evidence of both requirements into .
